It’s that time of year again and your Board of Directors needs your input on how the program went
during the regular season and how your team’s coaches handled their duties.  Parents and players should
work together to complete this form.  These  evaluations are confidential.  Coaches will be shown the
results, but not any player-identifiable or signed evaluation forms.  WE ASK ALL PARENTS TO PLEASE COMPLETE A FORM SO WE HAVE A TRUE EVALUATION OF THE PROGRAM AND COACHES.
